Definitely both issues. "Zero body fat" training taken to a mad extreme. She's all sinew and muscle!

She's pretty eccentric in terms of her bod, Madge. Her long-ago husband, director Guy Richie, said the marriage became untenable in part because she mummified herself every night head to toe in saran wrap, the better to keep herself basted in cold cream. True story!
