Wow that is nice.
I had to make a new bout extension for mine too. Turned out pretty good, starting with aluminum rod and just working slowly with a vise and improvised forms.
For the cushioning, I used surgical tubing with the same ID. I thought it would cling like crazy and never slip on. But I used some dish soap on the rod and inside the empty tubing, and "squish" it slipped on. Not black like the original, but fine for me.

Fortuantely, I have a second BBII, so had one to copy from. No heating involved. Just worked the aluminum rod slowly with help from a vise and plastic rod with the right diameter. Did use a piece of stainless tubing that just slipped over the rod, and kept the free end of the rod strait while I applied pressure to it. Just used a file held horizontally with both hands to file the flat the knob screws against, again just working slowly
Surgical tubing slipped on easily once I used dishsoap on the rod and inside of the tubing.
