Looks like a Bass VI, which I believe Jack played pretty frequently in Cream.

Only on the first album, and with Graham Bond(with Ginger drumming)before that…

Because of a botched custom paint job on the VI, he moved on to a Danelectro longhorn for the Disraeli Gears sessions, then finally settled on the Gibson EB-3.
