I'd have given him the benefit of the doubt. Perhaps he's in chronic pain and clueless. I'd have offered him my place in line, and let him know that I was doing so in recognition that he has some sort of condition that prevents him from waiting in line like the rest of us.
It's worth it to me to have to wait a little longer because: A. He may have a legitimate issue, or B. If he doesn't he'll still have to acknowledge me and give me an excuse, (or tell me to F-Off).
Either way, I come out of the deal feeling good about myself. In case A. I showed respect to someone who deserved it, or B. I have a funny story to tell everyone about.
