Is Brian the person I should contact at GK about this out-of-phase issue? I sent him an e-mail about a month ago and didn't get a reply.

I'm pretty sure my MB210 + 210MBE has been suffering from this out-of-phase since I got it about a year ago. I always just thought I was just crazy that I heard a lot less volume with the extension cab plugged in than without.

If you don't want to mess with the cab; you can make a out-of-phase /speaker cable to use until it fix. Make use you mark the cable so not to use it on another amp.

Yes, if you are talking about the combo. I have two. Still haven't gotten around to having my noisy tweeters fixed (note to self - band name: The Noisy Tweeters) but when I do they will be perfect (for me).
