As far as setup goes, you should do whatever feels comfortable and gets you the sound you want. My strings are set relatively low from the nut to the end of the fingerboard. I had to tweak the truss rod in order to achieve the lower string height at the "12th fret" marker.
The magnetic pickups should be positioned so that each string is balanced in output when you're soloing the magnetic pickups... unless you want the D string to be significantly louder than the E for some reason.
