Perhaps tame by comparison in this thread, but in 1987 when I was a teenager...freaked me the hell out.

Great album, one of my faves; I’m looking forward to the upcoming tour, hope it stays in place.
First one I thought of. And it's totally incongruent with the music...
Anything by Seth Siro Anton; these 2 ones are personal favorites

Gieger(sp) always has fascinated me. Especially the Alien series. I did not know he had done a album cover. Good for him and the Artist(s)

EP album cover, possibly from the band Exploited. Color photo of a German WWII tank captain, perched in the turret of his tank, with his face literally blown off. His eyeballs remained (the especially creepy factor), and his uniform was mostly intact. Obviously he was doing recon when a large munition exploded right in front of him. (Maybe an aerial bomb? Dude had no idea it was coming).
I Googled but couldn't find it. That's probably a good thing... not sure it would have been permissible here anyway. I originally saw the CD in the used section of a record store in Baltimore.
Not sure creepy is the word, but it was unforgettable.

Just about every Opeth album cover has a creep factor...

For a blues band in 1970 Savoy Brown's Looking In was pretty creepy. Today it is pretty tame.
